There is ongoing research investigating patients who progressed on prior immunotherapy regimens and received cabozantinib (Cabometyx). Those patients experienced a beneficial response with cabozantinib compared to everolimus, states Choueiri.

There is potential for cabozantinib to be an option in the frontline setting for patients with RCC, states Choueiri.In the results of the phase II CABOSUN trial, cabozantinib showed superiority in progression-free survival (PFS).

It could become an option for patients with intermediate and poor-risk RCC that are completely untreated and systemic therapy naïve, explains Choueiri.

Sunitinib (Sutent) remains a standard treatment in the first-line setting and is used as the control arm for multiple phase III trials. Pazopanib (Votrient) is a frontline standard therapy that is similar to sunitinib but may be better tolerated depending on the scheduling, states Choueiri. 

There is a subgroup of patients that have certain mutations that are showing high response rates to everolimus (Afinitor), explains Choueiri.

In the largest PRCC trial to date, savolitinib, a specific MET-only inhbitor, was evaluated in this patient population. Although little is known about the biology of PRCC, says Choueiri, a subset of patients harbor an alteration in the MET gene—MET mutation, MET amplificiation, and others.

Savolitinib was generally well tolerated in MET-driven patients, but further clinical investigation is warrented, says Choueiri.
